Melatonin and Jet Lag: What the Research Has Established

Jet lag is one of the most studied applications of melatonin. Here's what the randomized controlled trials have found and the mechanism behind how it works.

The Circadian Mechanism of Jet Lag

When you fly across multiple time zones, your circadian clock doesn’t immediately reset — it remains synchronized to your departure time zone while your external environment shifts.* This creates a misalignment between internal and external time that manifests as the cluster of symptoms called jet lag: impaired sleep, cognitive slowness, gastrointestinal disturbance, fatigue, and mood changes.*

The circadian system re-entrains at approximately 1–1.5 hours per day without intervention — meaning crossing 8 time zones takes roughly 5–8 days to fully resolve naturally.* Melatonin can accelerate this re-entrainment process by providing an external timing signal that shifts the circadian clock in the desired direction.*

What the Research Has Found

Jet lag is the application for which melatonin’s evidence base is most robust and consistent:*

  • Cochrane Review (Herxheimer & Petrie, 2002): Reviewed 10 randomized placebo-controlled trials and concluded melatonin was “remarkably effective” for preventing or reducing jet lag, with greatest benefit seen after crossing five or more time zones.* The reviewers noted that eastward travel (which requires phase advance — a more difficult circadian shift) showed the strongest benefit from melatonin.*
  • Dose comparisons: Research comparing 0.5mg to 5mg found no significant difference in jet lag relief outcomes, though the 5mg dose was associated with faster sleep onset.* Very high doses were not studied for jet lag specifically.*
  • Direction matters: Eastward travel requires the clock to advance (wake/sleep earlier); westward requires delay (wake/sleep later). Melatonin’s phase-shifting direction depends on when it’s taken — the appropriate timing differs for east vs west travel.*

A Note on Melatonin Selection for Jet Lag
