Melatonin Supplement Label Accuracy: What the Research Has Found

A 2017 study found that over 70% of melatonin supplements deviated substantially from their label claims. Here's what the research found and what it means when choosing a supplement.

In 2017, a study published in the Journal of Clinical Sleep Medicine by Erland and Saxena examined 31 commercially available melatonin supplements and found that more than 70% deviated substantially from their label claims — ranging from 83% below the stated dose to 478% above it. What the Study Found

Erland and Saxena (2017) tested 31 melatonin supplements purchased from retail locations. Key findings:

  • 71% of the products tested had melatonin content that deviated more than 10% from the label claim
  • The range of deviation was -83.3% to +477.7% — meaning some products contained less than one-sixth of the stated amount, while others contained nearly five times the stated amount
  • 26% of the products also contained serotonin — a controlled substance in some jurisdictions — not listed on the label
  • The actual melatonin content did not correlate with stated dosage, lot number, or retail source

This study was not uniquely damning of melatonin specifically — it reflects a broader dietary supplement industry quality problem that has been documented for multiple supplement categories. But melatonin is particularly vulnerable to this problem because:

  • It’s hormonally active in very small quantities — errors in either direction have real consequences
  • It’s widely available without quality oversight at the retail level
  • The dose-response relationship is non-linear — more is not simply more

Why Label Accuracy Failures Happen

Label accuracy failures in dietary supplements are primarily a manufacturing quality problem. Without rigorous finished-product testing before release, there is no mechanism to catch errors that occur during production. The most common sources of deviation:

  • Blending inhomogeneity: Melatonin, a potent active at milligram quantities, must be thoroughly blended with inactive ingredients. Poor blending creates “hot spots” and “cold spots” within a batch — different units from the same production run can have substantially different melatonin content.
  • Raw material variability: Incoming melatonin raw material that hasn’t been tested for potency may itself deviate from specification — errors compound through the manufacturing process.
  • Degradation: Melatonin is sensitive to light and moisture. Products that degrade during storage lose active content — a product that tested correctly at manufacture may have substantially different content by the time it reaches the consumer.
  • Lack of finished product testing: Without systematic testing of each production batch, errors go undetected and unreported.

What cGMP Manufacturing Addresses

21 CFR Part 111 (cGMP for dietary supplements) specifically addresses these failure modes. Required elements include identity testing of incoming raw materials, documented blending procedures with in-process controls, finished product testing before release, batch records with full traceability, and retention samples for post-market testing.
